One would need to actually build a guillotine. Even if you have some DIY skills when you’re at your wits end you probably don’t have the energy and willpower to build it. It’s also a very gory method and it’s unsure how long the severed head is conscious. I think it’s no surprise it’s not used that frequently as a sui method.
